Dr. Montgomery: In my experience we will occasionally try the epidural steroid injections. That would typically be in a group of patients having more neurologic symptoms, perhaps having radiating pain into their shoulders or arms, or some numbness and tingling. Maybe we're concerned about some nerve root compression. I have not had as much experience with the facet injections . Our pain doctors, our physiatry colleagues, who do a lot of pain management, would try things like nerve root blocks or trigger point injections . I have not seen as much of the facet injections in the cervical spine as I have in the lumbar spine or the low back.

This widely prescribed dog steroid is available in tablet form or as an injection, known commonly as Depo-Medrol. A potent anti-inflammatory drug, Medrol relieves itching quickly and can typically be given at lower doses than other corticosteroids. Medrol tablets are small and white, containing 4 mg of medication per pill. Your veterinarian will advise you on the right dosage for your dog. Depo-Medrol is typically administered as a single injection that lasts around 6 weeks. It may be recommended for one-time use or to treat dogs with seasonal allergies. Like other prescription steroids that impact the immune system, Medrol can increase susceptibility to illness and infection. To help reduce such risks, you may consider keeping your dog indoors or away from potential hazards while he is taking Medrol. Corticosteroids also affect the body's water and sodium levels, which can sometimes create side effects like water retention, weight gain and increased blood pressure. Because Medrol is given at lower doses, these side effects are typically minimized. Use of Medrol is generally not advised in dogs with tuberculosis, Cushing's disease or peptic ulcers. Extreme caution is also advised regarding the use of Medrol in pregnant dogs.

Use with other corticosteroids could increase the risk of side effects. Antacids can cause glucocorticoid absorption. Anticholinesterase use in patients with myasthenia gravis can cause muscle weakness. Use with diuretics can cause hypokalemia as a result of excessive potassium depletion. Estrogen agents may increase the effects of depo medrol. Use with macrolide antibiotics might lead to an increase in glucocorticoid activity and longevity. NSAID type drugs are not to be given to patients taking depo medrol. Taking with ulcerogenic drugs may increase the likelihood of a GI ulcer. Live attenuated virus vaccines should not be given at the same time as depo medrol, since the replication of the virus might be augmented.

Activated charcoal can act as an adsorbent (kind of a "chemical sponge") to bind toxins in the gut. If you use a preparation that is safe for people and use a smaller dose, it should be harmless.
If the problem was an insect sting, I doubt that there are any toxins in the gut that the charcoal would help with.
So many times we are unable to determine the source of the allergen.
Kenalog is a long-acting corticosteroid (cortisone) injection that stays in the body for weeks, where the dexamethasone lasts only a couple of days.
Antihistamines like benadryl are often helpful for something like an insect sting, even though most dogs don't get much relief from them in regard to allergic itching.

If the problem was an insect sting, I doubt that there are any toxins in the gut that the charcoal would help with.
